Community Health

Trellus Community Health programs improve the physical and mental health of our communities through outreach, health screenings, case management, and education, delivering equitable access to health resources and curbing the spread of infectious and chronic diseases.

We provide individuals and families with integrated services in client-centered, compassionate, and culturally appropriate ways. Some of the services offered include: HIV/AIDS testing, counseling, and prevention programs; home services and housing navigation for individuals with HIV; and health education including Hepatitis B Awareness and ADAP medication assistance.

Health Education

We offer free health education sessions and workshops which can be adapted to a variety of audiences on topics including:

  • HIV Prevention
  • Living with HIV
  • Men’s Health
  • Mental Health
  • Nutrition
  • Parenting
  • Reproductive Health
  • Visiting Your Doctor
  • Women’s Health and more

Housing Navigation

Our Housing Navigation Program assists people living with HIV with:

  • Identifying housing needs
  • Access to housing waitlists and resources
  • Applying for financial assistance related to housing
  • Education on housing options
  • Goal setting

DRS Home Services

Our home services program for individuals diagnosed with HIV provides personal assistants to help clients with activities of daily living. To be eligible, applicants must:

  • Be diagnosed with HIV/AIDS
  • Currently be a resident in the state of Illinois
  • Have a valid state identification card, valid driver license, or passport
  • Meet the criteria of the Department of Rehabilitative Services eligibility checklist
  • HIV Prevention, Testing & Counseling

Trellus offers a variety of programming to help with education and prevention of HIV. We also provide free HIV testing and safe sex kits to our clients and the community.

ADAP Medication Assistance

We assist individuals with HIV and those who are low-income, uninsured, or underinsured with applying for financial assistance to cover the costs of their medications. Additionally, we assist individuals with accessing related HIV care and financial resources.
